| Medicaid Case Management for Women & Children Case management services are provided to assist eligible clients in gaining access to necessary medical, social, educational, and other services, encourage cost-effective health and health-related care, discourage over utilization or duplication of services and make appropriate referrals to providers. Case managers provide the necessary coordination with providers of services when these services are under by a client. Lay Case Worker Program The Monica Lamb Wellness Foundation, Inc. has partnered with the Texas Children's Health Plan to pioneer a local outreach initiative designed to educate mother's and infants in an effort to improve health statistics for pregnant women and infants. This program is unique as a case management initiative in so far as the primary providers are Lay Case Workers. |
